The U.S. Census Bureau is streamlining operations to conserve taxpayer time and money. This includes moving from paper-based data collection operations to online-based data collection operations. Beginning with the 2014 ACES, data from employer companies and nonemployer companies will be collected primarily through electronic reporting via Centurion (our secure online reporting system).

To obtain a worksheet to compile your data before reporting online, click Survey Log in, enter your user ID and password, and select the link under the "Worksheet" column on the main menu. Please make note of any special instructions listed either on the main menu or on your worksheet.

Use the links on the left to view the current forms used to collect data for the survey. All forms are in Adobe PDF file format and are for informational purposes only.
