1. Are any reports generated from the data collected in the Report of Organization?
This survey is taken primarily to assure full coverage and high quality of other statistical programs, and does not routinely provide data products for public use.
Please note however, this survey is a major source of information for the County Business Patterns reports and as a resource in responding to requests for a variety of special reports and reimbursable tabulations.
Survey results are available to the Census Bureau about 8 months after each reference year and are used throughout Census Bureau economic data program operations.
2. Can I combine all my data?
No. Separate data are required for each establishment or place of business operated during the previous year. To publish meaningful data for all geographic areas and economic acitivities, it is necessary to obtain information for each of your locations.
3. Can estimates be used to complete the report form?
Yes. A good estimate is acceptable if book figures are not available for specific items. We do not expect businesses to incur additional expenses to develop information not available from their records.
4. Changes that were made last year are not present in this year's form. Why?
If your data was submitted past our processing date, changes may not have been made. If you need further information, please contact us.
5. How can I get a copy of the data my company submitted last year?

An annual mail-out/mail-back survey of some 50,000 multi-establishment companies.
The data is used to update the Census Bureau's list of businesses and provides key source data for County Business Patterns and other statistical series.
Annually since 1974; survey coverage and content vary during the census-related five-year program cycle. Data collection begins in late December of the reference year and responses are due within 30 days of receipt.
For duplicate locations, mark "other" in the status column and write "duplicate" in the remarks section.
Report all employees, both full- and part-time and employees working under a contractual co-employment arrangement whereby employees are provided by a professional employer organization. Include as employees all persons on paid sick leave, paid holidays, and paid vacations; include salaried officers and executives of incorporated firms.
Exclude any temporary employees brought on for a limited duration to supplement the permanent work force (only if they are employees of the temporary help company). If any employees are provided by a professional employer organization, please indicate this in the remarks section.
Payroll data should be entered in thousands. For example, if the figure is $114,796.43, enter 115.
If your company discontinued operations before 2001, please return the report with an annotation in the remarks section that your company did not operate in any part of 2001.
Yes. Complete the form up to the point the company was sold. Be sure to complete the new owner information.
An economic unit comprising one or more physical locations under common ownership or control.
A single physical location where business is conducted or where services or industrial operations are performed. (For example: a factory, mill store, hotel, movie theater, mine, farm, administrative office.) If distinctly separate activities are performed at the same location, and if there is significant employment in each acitivity, then each activity should be treated as separate, (For example: construction activities operated out of the same physical location as a lumber yard.),
except Agricultural Production as explained below.

All multi-establishment companies and their establishments that have a payroll, except companies engaged exclusively in agricultural production.
The purpose of this questionnaire is to obtain an accurate and up-to-date list of your establishments, i.e., separate business locations that were in operation or new plant(s) under construction during part or all of the survey year. This list should include establishments of your company and each of its subsidiaries except those reported on the MA-1000 form.

Yes. Importing data into the software must be done according to the importing format. Detailed instructions on importing data is available in the software's help system.
You can only remove stores that you added. Any other store you would like removed should be marked closed, sold, or other (with an explanation) for the status, and it will be removed from your file for the following year.
Yes. You can sort the spreadsheet as long as you do not change the column designations.
No. The software is provided to you at no charge.

You may submit your data using the CSAQ software via the Internet, diskette, or modem.
Microsoft Internet Explorer or Netscape Communicator versions 4.0 or higher are required.
Computerized-Self-Administered Questionnaire. A CSAQ is an electronic rendering of a questionnaire that controls the flow of survey questions. It provides interactive help and performs various edit checks as you complete the questionnaire.
Close the software entirely, re-open it and try printing again.
No. The data will be printed in the order shown in the software.
No. Because of the mandatory reporting requirement, the Census Bureau cannot excuse your company from filing. For more information on mandatory reporting, see the legal requirements page.
Title 13 of the United States Code provides complete protection for all reported information. Census forms, including copies retained by respondents, are immune from legal action. Also, Federal law specifically exempts the reports from the provisions of the Freedom of Information Act.
Your response is required by law. Title 13, United States Code, requires businesses and other organizations that receive this questionnaire to answer the questions and return the report to the Census Bureu.

Individual reports are seen only by sworn Census Bureau employees and only for the purpose of developing statistical totals and summaries. Census publications provide no information about an individual firm's operations.
Yes. Please be sure that all data, including the label information, are legible.
The form showing the old EIN should have the new one entered, and an explanation regarding the change in the remarks section. Report data for both EINs on the form with the correct EIN; staple the forms together and return to the Census Bureau.
Describe the physical location referencing distances from cities, major highways, shopping centers, etc. (i.e., 1/2 mile south of Boise on Route 728).
Please enter the new EIN on the form and explain in the Remarks section when the change occurred. Complete the form for the full year.
The EIN can be found in the second line of the address label immediately following the letters "EI".
If you were completing the form from its current location instead of downloading and saving it on your PC, the data was not saved. Download the files to your PC, choosing the "save to disk" option.

For 1995, the survey includes all 30,000 companies with more than 250 employees, and 20,000 smaller companies. Smaller companies are selected where separate administrative data indicate a probable organizational change and by a probability sampling procedure.
