Data are collected on different survey forms depending on the size and type of government. The 2009 survey forms are identified as follows:

•  E-7  [PDF, 393KB] – Major Special Districts and Agencies
•  E-9  [PDF, 374KB] – Police Protection Agencies

The content of the survey form has stayed consistent within the following years with the exception of dates and minor changes. Historical forms are available below. Any major changes that were made are listed below the set of forms. Please note all files are in Adobe PDF format.

• 2001 – 2008 Forms:

• Added the E-5 Municipality and Township form and changed detail collected on the E-6 School Systems form
• Employment information was only collected on the G-30 survey form for the 2007 Census

• 1997 – 2000 Forms:

• Changed the reference date from October 12 to March 12

• 1992 – 1997 Forms:
