Data are collected on different survey forms depending on the type of government and pension plan.

Historical Survey Forms
The content of the survey form has stayed consistent within the following years with the exception of dates and minor changes.
In 2002, data collection changed from "Book Value" to "Market Value." Starting with the 2005 data collection, the survey form specified to report data for Defined Benefit pension plans only.
