Census 2000 Short Form Request

Would your organization like a copy of the Census 2000 short form? Will your organization be doing R&D to prepare for possible 2010 Census acquisitions? Interested parties can obtain three (3) paper copies of the Census 2000 short form while supplies last.

To get copies of the Census 2000 short form, please complete the following two steps:

  1. Request a set of three forms by sending an email to dscmo.2010.dris@census.gov. Requests will be honored on a first come, first served basis, while supplies last.

  2. Send a self-addressed, postage paid envelope to:
