Survey Legitimacy

  4. The upper right corner of the log-in screen should display the Office of Management and Budget (OMB) eight-digit number and expiration date. This eight-digit number should match the letter or emails you received which also contain this information. The bottom of the log-in screen should have a link for the “Burden Statement” which also contains the OMB eight-digit number.
