The Diet and Health Knowledge Survey (DHKS) questionnaire presents many questionnaire design challenges. The questionnaire collects data about peoples' knowledge and attitude about various health and nutrition related issues. The design of the responses scales is an important ingredient in determining the quality of the data collected. In addition, more practical issues also come into play in developing a questionnaire that is efficient to administer in the field. Literature addressing response scales and data quality is often not consisten in its conclusions. Furthermore, practical concerns such as ease of administration, are often left completely unaddressed. The objective of this study was to further examine some of the data quality issues with response scales as well as incorporate measures of more practical concerns.
