Sample ACS & PRCS Forms and Instructions

Browse sample American Community Survey (ACS) and Puerto Rico Community Survey (PRCS) forms (or questionnaires) in English and Spanish with instruction guides from 1996 through present.

  • The household form asks about the people at the address on the mailing label and their house, apartment or mobile home. 
  • The group quarters form is for people living in group housing facilities such as college/university student housing, nursing/skilled nursing facilities, barracks, and correctional facilities.
  • El cuestionario de hogares piden información acerca de las personas que residen en la dirección de la etiqueta. También piden información sobre la casa, apartamento o casa móvil ubicada en la dirección que se indica en la etiqueta. 
  • El cuestionario de alojamientos de grupo piden información acerca de las personas que residen en los alojamientos de grupo, como los alojamientos para estudiantes universitarios, centros de enfermería / enfermería especializada, cuarteles, y instalaciones correccionales.

Use a pen with blue or black ink, or a #2 pencil, to complete the ACS or PRCS form you got in the mail. If you have misplaced the ACS or PRCS form that you received in the mail, please call 1-800-354-7271.
