Browse sample American Community Survey (ACS) and Puerto Rico Community Survey (PRCS) forms (or questionnaires) in English and Spanish with instruction guides from 1996 through present.
Use a pen with blue or black ink, or a #2 pencil, to complete the ACS or PRCS form you got in the mail. If you have misplaced the ACS or PRCS form that you received in the mail, please call 1-800-354-7271.
The Instruction Guide is no longer being mailed out to respondents. It was discontinued for housing units in 2017 and for group quarters in 2021. However, you can still find these instructions on our website. For instructions on responding to the survey, go to our Get Help Responding to the ACS page, or to the For People Living in Group Housing page (for group quarters only).
